Community Learning Disability Nurse - Band 6

An exciting opportunity has arisen for an experienced RNLD to work as part of The Community Learning Disabilities team in Nuneaton covering North Warwickshire.
The role involves close collaborative working with multi-disciplinary colleagues in the community setting, managing own caseload, supervising others, completing assessments, raising awareness of the health needs of adults who have a learning disability, identifying unmet health needs, and providing specialist nursing advice.
The successful candidate will need to be proactive, self-motivated, and have experience of autonomous working, whilst also working under the direction of the community team manager.
Main Duties of the Job
The successful candidate will also be able to use current best practice, guidance, and legislation to gain the best outcomes for clients and their carers. They will work within the MDT, assist in improving links with primary care, provide support, information, and education to clients, carers, and providers, and deliver education to people with learning disabilities around health needs both on a 1:1 or group basis.

Job Responsibilities
Essential Requirements include:
* RNLD and current NMC Registration
* Excellent communication skills
* Experience of working with people with a learning disability in the community
* Driver with access to independent means of transport

Qualifications
* RNLD
* Diploma / Degree holder
* Evidence of ongoing CPD
Experience
* RNLD with a substantial post-registration experience at Band 5.
* Experience of working in multi-agency teams with multi-disciplinary colleagues.
* Experience of working as a community nurse with people who have a learning disability.
Other
* Must have an independent means of transport.
* Access to a vehicle to use for work purposes.
