We are excited to offer a permanent opportunity for a motivated learning disability nurse to join our Enhanced Support & Liaison Service (ESLS) who support adults with intellectual/learning disabilities and autistic adults who are at risk of hospital admission and/or need support to access mainstream mental health services.
You will be working across the Learning Disabilities and Autism Service Directorate, linking in with local community learning disabilities teams, social care services, adult mental health services and third sector organisations. You will be supported by a multidisciplinary team. This post covers the geographical area of Lincolnshire and some travel outside of Lincolnshire is expected. Our services are rated as 'Outstanding in Well Led' and 'Good' overall by CQC.
Main duties of the job
You will be responsible for the co-ordination of care needs, assessment, planning and implementation for evidence-based care to a defined group of service users and their carers within the service catchment area whilst under the guidance of senior staff. You will provide professional advice and support to other agencies and members of the multi-disciplinary team.
The primary purpose of the service is to provide an intensive home treatment service to people with a learning disability and/or autism who are presenting with acute mental health or behavioural and emotional disturbance to a degree where the risk they pose indicates that an inpatient admission may be required.
You will assess care needs and develop, implement and evaluate programmes of care for those who are experiencing acute episodes within agreed limitations.

Detailed job description and main responsibilities
* Responsible for the assessment, planning, implementation and evaluation of the evidence-based care required, including health promotion for named service users/groups.
* Responsible for the appropriate clinical risk assessment of service users contacting the service, providing effective advice, support, guidance and decision-making processes culminating in the coordination of any reviewed plan of care.
* Work with individuals and their carers to identify their related needs and circumstances; and enable them to understand, manage and where appropriate change their behaviour.
* To work flexibly across a 7-day-a-week service.
* Responsible for the reviews/case conferences/professional meetings in collaboration with members of the multi-disciplinary team.
* Following assessment to “work outwards” from the referral and potential immediate crisis and help the service user stabilise and help the carers and extended system feel more confident about how they manage the immediate situation and any continuing difficulties.
* Protect people from abuse, neglect and harm.
* Enable individuals to develop independent living skills and live in the community.
* Participating in Care and Treatment Reviews and Local Area Emergency Protocol reviews and the decision-making around future treatment options.
* Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing in line with current clinical guidance practice and policy.
Person specification
Qualifications
* RMN or RNLD or RN: 1st Level Registration (NMC) or degree or equivalent
* Evidence of specialised continued professional training in clinical practice
* ENB998/Flips or equivalent
* Willingness to undertake Approved Mental Health Practitioner training
Experience
* Knowledge and experience of PBS
* Understanding by the post holder of their responsibilities under the Mental Health Act 1983
* Understanding of the Mental Capacity Act
* Understanding of Safeguarding Adults processes
* Understanding Best Interest assessment process
* Experience of working with mental health issues
