Job overview
37.5 hours per week – Monday to Friday between the hours of 08:00 – 21:30 working long days and occasional weekends and Bank Holidays.
We are looking for a highly skilled and motivated Mental Health Nurse to join our Older Persons Hospital Liaison Team. To be successful in meeting the challenges involved in the role, you must be able to work autonomously and have strong clinical and risk management skills.
We are looking for a RMN with experience with Older adults or with an interest to work with Older adults. Part of the day to day role is to support the general hospital wards with Older Persons mental health, assessing, continuing care and planning of a safe discharge.
Main duties of the job
1. Provide a response to Mental Health Crisis in Emergency Departments and Inpatient Wards within one hour and to all urgent Ward referrals within 24 hours.
2. Complete a full biopsychosocial assessment and formulation and contribute to treatment and collaborative care plans.
3. Offer brief evidence-based interventions as inpatient or short-term outpatient follow-up.
4. Work within Doncaster Hospital Teams to reduce length of stay on general hospitals and improve follow-up care.
5. Provide advice and support to Scunthorpe General Hospital Staff regarding mental health care for their patients.

Qualifications
* Registered RMN
Work Experience
* Appropriate demonstrable experience of working with older people with mental health problems. Experience of working with carers and families. Evidence of ongoing CPD and evidence based practice.
* Minimum 2 years’ experience.
* Evidence of ability to work across team and organisational boundaries developing multi-professional and multi-agency partnerships. Hospital liaison experience. Experience of integrated working with physical health services.
Communication Skills
* Demonstrates advanced and comprehensive communication skills. Clear and concise command of English (written/oral). Ability to organise and carry out teaching practice. Basic IT skills/Computer skills. Positive presentation skills.
Personal Qualities
* Self-motivated, starter and finisher. Must be able to demonstrate ability to prioritise workload. Organised. Highly motivated. Flexibility.
