We are developing an Integrated Community model, focusing on new ways of working within integrated mental health teams to support people living in their communities with mental health challenges.
The role requires undertaking community-based interventions to work with people who have severe and enduring mental health conditions, particularly those who historically avoided contact with services.
The post holder will employ a wide range of skills, building on and developing their expertise and actively seeking advice and collaboration with issues that may not be readily within their sphere of competence.
The role will include working as a Care Coordinator, where you will have responsibility for managing a caseload of service users with mental health needs who have been referred to the team via a number of access points. As a further extension of this role, you will also be involved in Crisis management as part of the duty system and undertake the role of the duty nurse.
You will work from our community base facility, via phone/agile working or within the service user’s home.
Main duties of the job
You will be a Registered Mental Health Nurse, preferably with adult and community mental health experience of working with severe and enduring mental health service users. You will be responsible for care coordinating service users with complex long-term mental health problems who find it difficult to engage. You will have experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems and have current knowledge of NHS policies and practices in mental health. You will participate and support the CMHT duty system and be responsible for managing the mental health needs of service users referred to the team.
The successful applicants will also have previous experience of the Recovery Model, a good working knowledge of psychotropic medication and effectiveness, plus the ability to deliver a range of therapeutic interventions. In addition, the post holder will demonstrate the ability to undertake and manage clinical risks and develop robust clinical management plans for Service Users within the service. You will support the team manager and clinical lead to adhere to and deliver high-quality service and have in-depth knowledge of national quality indicators.

1. NMC - RMN Registration
2. Experience of working within mental health services
3. Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006 and other relevant legislation
