Who You Are You are a social work professional with a strong understanding of the Mental Capacity Act 2005 and its application in practice. You have excellent assessment skills, the ability to engage effectively with service users, and a focus on promoting the wellbeing of older adults and their carers. You have experience in a collaborative, strengths-based, and outcome-focused model of practice, and you can develop and sustain effective multi-agency working relationships. What the Job Involves This exciting opportunity in the Older Persons Team South involves working with individuals aged 65 and over, across the Hay and Talgarth, Brecon, Crickhowell, and Ystradgynlais localities. You will undertake assessments and reviews, and support individuals in finding appropriate solutions during personal challenges as well as for long-term care planning. The role includes safeguarding individuals from abuse and neglect and collaborating with other agencies, the third sector, and health colleagues to provide proportionate and timely responses when needed.
