Work location: Alton with travel across the surrounding area, hybrid working (home and office based)
Hours per week: 37 (part-time hours will be considered)

The Role:
Join us as a Case Worker in our Older Adults team, supporting individuals aged 65 and over. You’ll use a strengths-based approach to assess an individual’s needs and provide them with support and guidance to maximise their wellbeing. Strengths-based practice focuses on people’s resourcefulness, strengths and abilities to enable them to develop the best strategies to face life’s challenges and live independently and safely in the community. Supported by experienced colleagues and qualified social care and health care professionals, you’ll use your effective relationship management and negotiation skills to collaborate with individuals, their families, providers and other health care professionals.

Travel Requirements:
You will be required to travel to various locations within Hampshire or other local authority, and it is essential that you have a current drivers’ licence and access to a reliable vehicle on a regular basis.
