To carry out pre-defined care plans with patients in their homes, as directed by a senior support worker or qualified therapist for a range of patients promoting an enablement ethos where appropriate, ensuring that patients safety and care is prioritised at all times. To report back to the allocated senior support worker, clinician or therapist, and document effectively care tasks undertaken, progress made, outcomes of intervention and any concerns regarding patients health, wellbeing or ability and to discuss when modification of care plans may be necessary as part of a structured clinical supervision system. To utilise basic health observation techniques to monitor patients health status as appropriate and directed by senior staff, reporting back findings. To ensure all aspects of work are carried out to a high clinical standard and be accountable for this through the senior staff. To maintain confidentiality on all matters relating to patients/families adhering to Trust policies. Communicate effectively with patients / families / other members of the multi-disciplinary team to ensure prescribed plan of care is delivered.
