Collaborative Community Care (CCC) is home to the services that lead on frailty. Be that rehabilitative, recuperation after injury or illness, assessment for long term needs or palliative care our services touch upon every aspect of frailty care. The services in the division are Outpatients and Theatres, Community Hospitals, Adult Community Therapy and Community Hospital Therapy. You will be leading a select combination of these dependent on previous experience and expertise. With LCHS recently entering into a 'Group' model with colleagues in United Lincolnshire Hospitals Trust as well as the implementation of the Lincolnshire Frailty Strategy there is significant opportunity to evolve our existing services and enrich the offer to patients. Main duties of the job This role will focus on the delivery of care in community, hospice and ward based settings managing nursing and therapy teams.
