Stafford Town Primary Care Network is seeking an enthusiastic, forward-thinking newly qualified (within the last two years, without a substantive post) GP to join our team.
You will have the unique opportunity to work across our 4 practices. You will be assisting the 4 practices to offer a range of routine GP appointments, alongside Same Day appointments to assist with day-to-day activity. The expectation will be to undertake 12 clinical contacts per session or 24 clinical contacts a day alongside a home visit.
Main duties of the job
You will play a vital role in delivering patient care.
Key responsibilities include:
1. Conducting routine and urgent appointments, both face-to-face and via telephone, ensuring high-quality care.
2. Prescribing according to local and national guidelines.
3. Handling home visits for routine and emergency cases.
4. Managing administrative tasks, such as letters, results, referrals, and emails, in a timely manner.
5. Making appropriate referrals and managing related admin.
6. Checking and signing repeat prescriptions to maintain care quality.
7. Providing cover for colleagues in emergencies, considering individual commitments.
8. Participating in on-call sessions as per pro-rata hours.
9. Collaborating with other health and social care providers to deliver integrated care.
10. Maintaining accurate consultation records using the EMIS clinical system.
11. Offering clinical support to colleagues, helping the team meet high standards in patient care and QOF/Enhanced Services targets.

Clinical responsibilities:
In accordance with the practice rota, as agreed, the post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols, developing care plans for health.
Providing counselling and health education.
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
Recording clear and contemporaneous consultation notes to agreed standards.
Person Specification
Knowledge and Skills
* Proven ability to evaluate the safety and effectiveness of own clinical practice.
* Understanding of evidence-based practice.
* Evidence of working autonomously, with some support and supervision.
* Ability to effectively triage, assess and diagnose.
* Requesting pathology tests and processing the results, advising patients accordingly.
* Assessing and managing acute deteriorations/exacerbations of Long-Term Conditions.
* Understands the importance of evidence-based practice.
* Ability to assess and manage patient risk effectively and safely.
* Broad knowledge of clinical governance.
* Ability to work within own scope of practice and understanding when to refer to GPs.
* Excellent communication skills (written and oral) including recording of accurate clinical notes.
