Job Description:
We are pleased to be able to offer a post as a band 6 Senior mental health practitioner with the Teignbridge OPMH team.
Teignbridge OPMH provides community mental health services to people over the age of 65 with functional mental health problems and people of any age with an organic mental health problem.
Teignbridge OPMH is an established team that works to a multi-disciplinary and multi-agency model.
Main duties of the job
The post holder will deliver high quality comprehensive mental health services. The role involves managing a defined caseload with a focus on providing expert assessment and treatment for people with complex mental health difficulties and their carers. This will include key working, design and co-ordination of treatment packages that are personalised and evaluated. The liaison with a wide range of statutory and non-statutory agencies is central to this role.
The post holder will contribute to the clinical leadership of the team, providing supervision for other members and participating in team and service development, and evaluation.

Job responsibilities
* Contribute to the teams engagement with a wide range of stakeholders including staff, service users, their carers and other statutory and voluntary sector agencies.
* Impart highly complex and sensitive information to patients and families/carers regarding assessment, care planning, treatment and review.
* Ensure the provision of consistent well-coordinated care within multi-disciplinary teams.
* Liaise with external agencies involved in the provision of care e.g.: voluntary and private sector, police, social services, housing, employment etc.
* Participate in and chair clinical meetings, including CPA and non-CPA reviews, professional meetings, and clinical risk meetings.
* Demonstrate the ability to communicate in a way that is empathic and reassuring while employing negotiating techniques to gain cooperation from complex individuals.
* Facilitate fair access to social care services and promote social inclusion regardless of age, ethnicity, gender, sexual orientation or disability.
* Undertake complex risk assessments and develop risk management plans.
* Ensure the continuous re-evaluation of individuals' needs and clinical risk situations.
* Organise and facilitate complex meetings related to CPA and risk assessments.
* Plan and organise individuals' packages of care and ensure timely reviews.
* Deliver services using a recovery and strengths approach.
* Provide specialist advice to referrers and other multi-disciplinary staff regarding the care of individuals.
* Ensure safeguarding measures are in place for adults and children.
* Maintain up-to-date clinical records in line with Trust standard operating procedures.
Person Specification
Qualifications
* A professional mental health qualification and registration e.g. RMN, BSc/Diploma in OT, SROT, Social Work Degree or equivalent, Community Nursing Degree.
* Non-Medical Prescriber and/or another recognised therapy qualification.
* Postgraduate qualification relevant to specialism.
* Training to level 3 in Safeguarding Adults and children and young people: roles and competencies for Health Care staff.
Experience
* Substantial experience of managing an extensive and complex caseload of clients with a range of complex mental health presentations.
* Experience working within a community mental health setting in either the statutory or voluntary sector.
* Experience providing clinical supervision.
* Experience working in a multi-disciplinary and multi-agency environment.
* Participation in user involvement in services.
* Experience delivering model-based psychological therapy.
