Mayflower Medical Group is looking to recruit a PCN Paramedic Practitioner to compliment the existing home visiting team.
Your role
You will focus on supporting the practices with managing urgent and unscheduled care, predominantly to our housebound patients. Your role will be to provide clinical assessment by remote (telephone or video) and face-to-face consultation, to diagnose, treat, refer or signpost patients/service users who present to the surgery with undifferentiated or undiagnosed conditions. To use advanced clinical skills to provide education to service users, promoting self-care and empowering them to make informed choices about their treatment. Alongside home visiting, you may also compliment the 'on the day' service, providing care for patients/service users who attend the surgery with undifferentiated or undiagnosed conditions relating to acute illnesses or minor injuries.
The post holder must be a registered Paramedic with postgraduate qualifications relevant to the position. Experience in General Practice and independent prescribing training would be desirable but not essential. More important is a positive attitude and expertise in engaging people; and the ability to work with a wide range of people to ensure all patients receive the best possible care and service.
You will be engaged with the Roadmap to Practice pathway to support your role as a First Contact Practitioner.
Main duties of the job
Responsibilities:
1. Work with our GPs and other primary care professionals in order to identify and manage a caseload of patients with specific needs, flagging any concerns to the relevant GP and referring to other health professionals as appropriate.
2. Act as an expert practitioner, demonstrating clinical competence and a sound knowledge base.
3. Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.
4. Provide clinical advice to Reception staff where required.
5. Ensure that patients receive high quality clinical care, delivered in a timely manner.
6. Ensure complete and accurate documentation of each and every patient contact.
7. Work as an independent practitioner and as part of the multidisciplinary and multi-agency teams, in order to ensure patients' needs are met.
8. Ensure that professional standards are maintained and within the guidance of the Department of Health and the Nursing and Midwifery Council (NMC)/Health and Care Professions Council (HCPC) and the College of Paramedics (COP).

* 5 years post-registration experience.
* Current Paramedic registration on the HCPC register.
