Job Title: Senior Community Healthcare Assistant

Main duties of the job
* Following appropriate induction, supervision and competency-based training; you will be able to work with patients in their own home or an environment appropriate to their needs, providing care as required.
* To assist the trained staff with admin tasks as directed.
* Works as part of the multi-disciplinary team to provide Nursing, social and palliative care, and any other components of the Out of Hours service (and others as they are developed).
* To complete office/computer-based patient notes and patient held notes with up-to-date information if required.
* To liaise with on call coordinator during out of hours, for support and advice or provide any relevant updates and changes that may affect the service.
* To communicate effectively appropriate information in an understandable format to patients, carers and other staff.
* Understand patients' right to choice, privacy and dignity and the need to treat patients as individuals.
* Responsible for time management and spending appropriate time on each visit ensuring the patient does not feel rushed or uncomfortable.

* NVQ level 1 and 2 in health and social care.
Experience and knowledge:
* A minimum of 6 months paid experience in a community environment within the last 2 years.
* Knowledge of community health and social care issues.
* Relevant experience that supports good communication skills.
* Experience working in Health and Social Care.
