The Role
Join Bolton’s Adult South and Central Neighbourhood teams, working closely with health colleagues to support adults and carers eligible for assessment and care under the Care Act 2014.
We focus on helping people live well at home through collaboration with Social Workers, Occupational Therapists, Nurses, and Mental Health Practitioners, delivering person-centred, strength-based care to protect the vulnerable and build strong, supportive communities.
Key duties include conducting assessments, care planning, reviews, Mental Capacity Assessments, participating in Best Interests meetings, and handling complex cases. The role also involves safeguarding, risk assessment, and coordinating resources from social care, health, and other agencies to support independence.
You will complete administrative tasks, attend case conferences, and represent the Council in court when necessary.

About You
The Social Worker role is key in ensuring the welfare and assessed needs of those whom we support are managed in a safe and inclusive way.
We adopt a strength-based approach to our practice and seek to provide a person-centred service. We are looking for resilient practitioners who can manage competing demands and prioritise work effectively. The role requires applicants who are confident decision makers and who can stay calm under pressure.
While in this role you will need to demonstrate commitment and enthusiasm to working closely with those with care and support needs, their families and carers, other professionals and your managers and peers.
You should hold a degree in social work, be registered with Social Work England, with evidence of continuing professional development and a strong understanding of the application of Care Act, Mental Capacity Act and Adults Safeguarding.
