This post is for a Band 6 junior practitioner within a 24/7 rapid response multi-professional team within Gateshead.
The post holder will work effectively within a multi-disciplinary/agency team attending to patients identified as requiring an urgent or rapid intervention to prevent deterioration in their health and wellbeing. In addition, there will be planned activity to provide hands-on care and holistic assessment to prevent avoidable admission to hospital, and/or long-term care and to maximise patient independence.
This post is designed to support the Rapid Response Senior practitioners and the Rapid Response Team Lead fulfil their objectives, ensuring the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients' health and wellbeing, within the community setting. Patients accessing the service will have complex needs and potentially numerous co-morbidities including frailty, palliative and EOL care needs.
This post will contribute to the safe delivery of a Rapid Response Service 24/7 for the patients of Gateshead, and will support the Senior Team in the safe and effective delivery of their role in unscheduled, intermediate care, palliative and EOL care environment.
Main duties of the job
As a Junior Practitioner you will be part of a community-based workforce providing specialised input as an independent and interdependent practitioner developing relationships across Gateshead, preventing unnecessary acute hospital admissions, enabling individuals to remain in their own homes for longer and prevent premature admissions into long term care facilities.
You will assist individuals and their carers to proactively manage and anticipate their future health needs, support self-care, develop appropriate plans, and work collaboratively with the wider multidisciplinary team, partner organisations such as NEAS, GP/ primary care, pharmacy services and wider Integrated Locality Team members.
Job responsibilities
* The post holder will support the team lead in delivering and evaluating high quality patient centered services in conjunction with the associated clinical teams to ensure this is incorporated within the Community Business Unit and Trust objectives, in line with Local and National Guidelines.
* Be responsible and accountable for delivery of an identified planned or unplanned intervention working as an autonomous practitioner within their scope of competence.

* Management and leadership of an identified team of staff within the rapid response service with accountability and responsibility for the safe delivery of all aspects of patient care within service portfolio.
* Deliver hands-on clinical care to a designated cohort of patients within Gateshead for a defined population of patients in an unscheduled care environment.
* Support the team leader in continuous service improvements and support the delivery of transformational projects within areas of accountability.
* Operational responsibility to lead and contribute to the service delivery to CIP/CQUIN indicators and other performance and quality indicators within the team as directed by the team lead.
* To demonstrate good leadership, motivate and support the team approach to delivering new innovative models of care for patients across pathways and across providers as defined by the community Services Business Unit and Team leader.
* Ensure systems and processes are in place to maintain patient safety in line with choose safer care program in collaboration with the Clinical Operational Managers Quality and Risk Managers and Clinical Lead.
* Ensure the experience of our patients and their families is understood within all community teams.
* Contribute to workforce development in line with team profile and National Nursing and AHP/Multi-professional guidance.
* Facilitate the development of clinical expertise and advances in Nursing and AHP practice within the team in collaboration with the Team Lead, Clinical Operation Manager and Clinical Lead.
* To ensure the development and maintenance of a learning environment within the multi-disciplinary team that is appropriate for the development needs of all members.
* To ensure all new staff undertake an induction process adhering to and following standards, competencies and ongoing education.
* To establish an effective network of communication. Encouraging a two-way dialogue and that ensures that all members of the team are fully informed of the Community Business Unit and Trust developments.
* To facilitate an environment where health promotion and informed patient choice is encouraged and developed to its maximum potential, within the available resources.
* To evaluate clinical practice within the area by monitoring standards of care and ensuring that nursing practice is audited, and research based and supports the clinical governance framework.
* To lead and facilitate an integrated approach to quality issues so that every member of the team contributes and is aware of their responsibility in this area.
* To facilitate and ensure a safe, efficient and effective pathway for patients using the service.
* To develop and participate in teaching packages for all grades of staff.
The post will be based in Gateshead. The post holder will be expected to travel to patients living within the borough of Gateshead and locally, if the patient is registered with a Gateshead GP.
The post holder will be expected to work flexibly covering a variety of shifts to meet the needs of the service 24/7.
