Job summary
We are looking for a dedicated Health and Wellbeing Coach tojoin our team. You will support patients, particularly those with long-termconditions, in managing their health and wellbeing. Using health coachingtechniques, you will empower patients to understand and manage their conditionsindependently.
Main duties of the job
1. Provide personalised care and wellbeing coaching to a cohort of patients.
2. Work closely with GPs and multidisciplinary teams to develop and implement patient care plans.
3. Participate in multidisciplinary team meetings to discuss patient progress.
4. Engage with community resources and initiatives to promote healthy lifestyles.
5. Manage a caseload independently, ensuring patient outcomes are met.
6. Support quality improvement projects across the PCN.
