Clinical Pharmacist - Tonbridge Primary Care Network
Join West Kent Primary Care and become a vital member of our Tonbridge Primary Care Network team as a PCN Pharmacist. We are looking for Pharmacists with acute, community or primary care experience who are passionate about developing their skills and knowledge and delivering high quality, friendly patient services.

If you do not have experience in this area of care but you can demonstrate a robust clinical background, we will support your learning in this area.
The PCN Pharmacist will be based at one GP practice and encouraged to build a strong working relationship with the practice team, GP supervisor, alongside their PCN colleagues. The role offers excellent professional development opportunities with a requirement to register on an 18-month Centre of Pharmacy Postgraduate (CPPE) course followed by an independent prescriber course which will be fully funded and supported with study leave.
Main duties of the job
The Practice-based Clinical Pharmacist will improve the quality and safety of medicines management. The pharmacist will provide specialist knowledge and expertise in the use of medicines based on the local demographic needs of the patient population. They will have regular contact with patients and help to reduce the workload of GP prescribers in the practice. They will support the management of patients with long-term conditions.

*Due to the geographical location, it would be beneficial to have a full driving license as travel will be required to practices across the PCN area.
Job responsibilities
* Work as part of a multidisciplinary team to manage medicines on transfer of care and systems for safer prescribing.
* Perform face-to-face structured medication reviews of patients and prescribe alternative medications, help reduce waste, and promote self-care.
* Lead on medicines optimisation within the PCN to meet the needs of QoF and other targets whilst improving quality for patients.
* Provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship.
* Provide specialist expertise to help address public health and social care needs of patients to tackle health inequalities, referring onto social prescribing if required.
* Contribute to practice financial stability through optimisation of medicines related targets in QOF delivery.
* Carry out Enhanced Access sessions, which may include some evening and weekend work.
* Undertake face-to-face reviews of patients with specific chronic conditions that fall within their competency (such as asthma reviews).
Person Specification
Experience
* Previous experience as a community / acute / primary care based pharmacist
* Experience in providing clinical advice and expertise on medicines to colleagues and patients
* Knowledge of structured medication reviews and producing recommendations for prescribing and monitoring
* Knowledge of medicines management at point of hospital discharge including patients living in care homes
* Experience of managing long-term conditions in a clinical setting
Qualifications
* University Degree in Pharmacy Sciences
* Registered with the General Pharmaceutical Council
* Registered Prescriber
* Further education in long term conditions
Other
* Knowledge or Use of the clinical system EMIS
