An exciting opportunity for a Band 5 Dietitian (permanent and full time) to join our community Nutrition and Dietetics team. This post is ideal for someone who is enthusiastic and self-motivated, with an interest in developing their knowledge and skills in nutrition support across a diverse and vibrant area in central London.
Our team has adopted and is embedding a Food Based Approach to managing malnutrition, delivering on personalised care and health coaching, and actively supports primary care with the appropriate prescribing of oral nutritional supplements.
The service includes a successful and proactive team of dietitians and dietetic assistant practitioners. We are a supportive and innovative team that continuously seeks to adapt and improve the service we provide as well as a strong focus on the health and well-being of our staff.
Our team purpose statement is 'We are committed to enabling our patients to achieve their nutrition and hydration goals through providing evidence-based dietetic care.'
The team is predominantly hybrid working. The post will involve a combination of face-to-face clinics, virtual clinics, and home visits in the boroughs of Hammersmith & Fulham, Westminster, Kensington & Chelsea.
The post holder will be part of a wider network of Dietitians throughout the Trust. There is a strong emphasis on learning and development and expect all staff to participate in Quality Improvement.
Main Duties of the Job
Your own caseload of patients who require nutrition support. This can be face-to-face at health centres, at home or in care homes and virtually (via telephone or video) as per service users' requirements.
A caseload of HETF patients in care homes (after the relevant Band 5 competencies have been completed).
Actively partaking in quality improvement projects and/or audits. Supporting student training.

Qualifications
* Degree in Nutrition and Dietetics.
* Health Care Professions Council Registration.
* Membership of the British Dietetics Association.
* Member of Special Interest Group.
Experience
* Experience of working with patients with one or more nutrition related conditions e.g., Diabetes, cancer, functional gut disorders, malnutrition.
* Experience in managing patients requiring nutrition support for both oral and enteral nutrition.
* Ability to present complex information to large groups e.g., presentations to patients or health care professionals.
* Understanding of the importance of effective multidisciplinary working and understanding team dynamics.
* Experience of Quality Improvement, audit and/or research.
* Experience in training/teaching health care professionals.
* Development of evidence based dietetic resources.
* Experience of using behaviour change techniques in clinical practice.
* Experience of working in a community dietetic team.
Skills
* Ability to apply core Dietetic skills across a range of clinical conditions.
* Ability to undertake anthropometric measurements e.g., weights, heights, mid upper arm circumference and interpret biochemistry to assess nutritional status.
* Ability to communicate complex medical conditions in sensitive and easily understandable language.
* Excellent oral and written communication skills with people from a variety of backgrounds e.g., via advocacy/interpreters.
* Knowledge of and commitment to equal opportunities.
* Basic IT skills - Microsoft Word, PowerPoint, MS Teams, Excel.
* Ability to travel to see patients in their own homes, care homes and clinics using public transport.
