All Age Continuing Care (AACC) haveopportunities for band 6 Senior Practitioners to join our Clinical Bank.
The post holder will play a key role across the CHC service,working within the remit of the National Framework for Continuing Healthcare,to deliver an effective, efficient and equitable service to the people andpopulations of Sussex.
Prior experience of CHC would be beneficial, but is notessential for this role, as you will work within an experienced and supportiveteam who will support you to develop the relevant knowledge required. The postholder should however have experience of working within health or care, andwill need excellent verbal and written communication skills, as you will berequired to work closely with patients, families, carers and other externalpartners.
Main duties of the job
As a Senior Practitioner, your role will include delivery of avariety of functions across the CHC service, including: Assessment, Reviews,Care Planning and Case Management.
You will also have the opportunity to work across other areas ofCHC, including audit, appeals, retrospective assessments, complaints,assessment verification, and many more.
You will be supported to develop the necessary skills andexperience to work across the whole pathway, safely, confidently andeffectively.

Job responsibilities
As a Senior Practitioner, your role will include delivery of a variety of functions across the CHC service, including:
1. Assessment

You will also have the opportunity to work across other areas of CHC, including audit, appeals, retrospective assessments, complaints, assessment verification, and many more. You will be supported to develop the necessary skills and experience to work across the whole pathway, safely, confidently and effectively. NHS Sussex is strongly committed to your development, and through a structured 1:1 and appraisal system, will work with you to recognise and create a plan to meet your development needs.
The role will require travel on occasions, to carry out assessments and reviews. Access to transport is therefore necessary, be it personal or public. All CHC staff are expected to work flexibly, when required, to meet the needs of the service.

5. Experience of working in a health or care environment (including acute, community, mental health, local authority etc.)
6. A good understanding of the health and social care environment and roles and responsibilities within it.
Desirable
7. Negotiation, conflict management and influencing skills.
8. Involvement in operational processes and evidence of using a problem solving and forward thinking approach.
Skills and Knowledge
Essential
9. Skills in patient assessment interpreting both physical and mental health needs and formulating care plans to meet identified needs.
10. Demonstrates ability to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales.
11. Ability to address complex or sensitive issues in an organised and appropriate manner both verbally and in writing at all levels; written documents, spread sheets.
12. Knowledge and experience of working with Safeguarding issues and Policy guidance.
13. Knowledge of the Mental Capacity Act and Best Interests decision making.
14. IT literate and working knowledge and computer proficiency of all Microsoft Office packages ( word processing, spread sheets, e -mail and internet use). Intermediate keyboard skills.

15. Knowledge and understanding of current national and local policy/Framework Guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care.
16. Understand and be able to identify and support the Freedom of Information and Subject Access Requests as required.
Qualifications
Essential
17. Educated to degree level or equivalent experience in one of the following fields:
18. -Allied Health Professions (AHPs)
19. -Social Work
20. With subsequent up to date registration with the relevant regulatory body, including Health & Care Professions Council (HCPC) or Social Work England.
