Role Purpose:
The Stockport Mental Health Network Engagement Worker will provide a range of services to people aged 18+ across the Stockport area, who have previously been known to Secondary Mental Health Services. Support could include combating social isolation, health & wellbeing, leisure activities and community participation, informal advocacy and much more. You will aim to connect people to their communities to engage in opportunities to contribute to a fulfilled and meaningful life and maintain mental health whilst facilitating independence and a move away from traditional Mental Health Services, where appropriate.
The post holder will also work with the other members of the Pure Networks & within the community to meet the service objectives.
Core Responsibilities:
1. To plan and prioritise personal targets for each of the clients referred to the service.
2. To be an integral part of Stockport preventative services.
3. Support and match clients to suitable activities and opportunities as identified.
4. Creative problem solving.
5. Offer individual support to clients over relevant timeframes.
6. Update personal support plans for each client.
7. Liaise with partner organisations and the wider Voluntary, Community and Health Sector organisations within Stockport that may be able to provide additional support to clients.
8. Enhance people’s lives through enabling participation.
9. Maximise client independence.
10. Identify natural support networks either in the community or in people's own networks of support.
11. Work in partnership with all project partners as part the SMHN and Pure.
12. To meet agreed targets, and report on these at agreed periods.
13. To keep and maintain records of activity, both written and electronic, and produce regular monitoring information as required.
14. To participate as appropriate in the monitoring and evaluation of the SMHN.
15. To work as an active member of the Stockport Third sector attend meetings and contribute to the ongoing development of the service.
16. To undertake and contribute to appropriate training.
17. To participate in a formal programme of 1:1 meetings and annual appraisals.
18. To participate in conferences, courses and training events as required.
19. To maintain an up-to-date knowledge of relevant legislation and service developments and to adhere to all appropriate policies, procedures and guidelines.
20. To uphold, understand and adhere to all policies and procedures identified by Pure Innovations Ltd.

22. To show a willingness to raise any concerns with management.

27. To carry out any other duties as required by the Line Manager.
