We are seeking a Clinical Pharmacist to join a dedicated healthcare team in Perth. This role involves working collaboratively with GPs, nurses, and other healthcare professionals to ensure optimal patient care. Key Responsibilities: Manage and review patient medication plans. Provide expert advice on the safe and effective use of medicines. Conduct medication reviews and reconcile medicines post-hospital discharge. Educate patients on proper medication use and potential side effects. Support GPs in achieving QOF and prescribing targets. Requirements: Qualified Pharmacist with GPhC registration.
