A fantastic opportunity has arisen for a Band 5 to join our SDEC Unit at Buckinghamshire Healthcare NHS Trust. This is an exciting opportunity for the post holder to join a team at the forefront of innovative and transformational care provision, which best serves our local community. SDEC is a fast-moving environment offering same day assessment, investigation and diagnosis for patients well enough for their care to be managed from home. The service also offers daily treatments and reviews to those same patients. Referrals are taken directly from GP’s, Advanced nurse Practitioners in the community, Emergency department, Urgent treatment centre ( UTC) at Stoke Mandeville and Paramedics and other specialities.
Duties will include being the first point of contact for all patients and families and providing initial nursing assessment, observations and investigations.
Tracking and ensuring a smooth pathway for these patients during the day, liaising with the medical staff and all support services involved for each patient.
Ensuring follow up is organised and managing daily treatments for those returning patients.
• To support the Charge Nurse/ Sister to monitor the safety and quality of care for patients.
• To manage a group of Nursing Assistants and assign delegated tasks and responsibility as appropriate.
• To ensure the compliance of mandatory training and appraisal requirements for delegated Nursing Assistants.
• The post holder will teach registered and non-registered staff, including pre-registered and training posts.
• To participate in maintaining a positive working environment incorporating patients’ feedback to maintain and improve patient care.
• To act as an effective role model promoting excellence through practice.
