Community Mental Health Centre Support Worker
We have an exciting post available within Hywel Dda UHB Early Intervention in Psychosis team. We are looking for an enthusiastic individual to join our established service in the management and support of individuals experiencing first episode in Psychosis or are determined as being in at risk mental state.
We have two Band 3 HCSW posts to work alongside our team covering all 3 counties. Base will be determined following interview and will be dependent on where the successful applicant lives.
The EIP service within Hywel Dda is set to see several changes in our service delivery over the coming year including an expansion of our age range from 14 up to 35 years. We are utilising skills for the development and management of individuals receiving support for the management of Psychotic illness which include the principles of medication management, case management, developing psychological therapeutic links with individuals and their families.
Early Intervention in Psychosis (EIP) services help young people with a first episode of psychosis (FEP) in their recovery and to gain a good quality of life. Not providing these services significantly impacts on an individual's chance of recovery and increases the likelihood of them developing a lifelong illness. It is therefore critical that people who, given the right support, have the opportunity to achieve a good quality of life and play a meaningful role in society.
Main duties of the job
To provide a recovery-based, holistic and person-centred service to the service-user.
Group based on the guidance given by the Registered Practitioner and interventions independently as delegated by the Registered Practitioner as part of an overall plan of care, with individual service-users or in groups following agreed guidelines, protocols, care pathways and guidance of a team of professionals in accordance with Health Board policies.
To work with the Registered Practitioners to deliver an effective, efficient, therapeutic service, including lone working in a variety of settings e.g. service-user's home, wards, nursing/residential homes, clinics, leisure and day centres.
To promote equality, respecting the rights of individuals, maintaining objectivity and integrity, to empower service-users and carers to make informed decisions about choices available within the parameters of the care plan and available resources.
To contribute to the protection of individuals from abuse as defined by the social services well-being act (SSWBA) under Safeguarding. Enable service-user to access advocacy services.
To feed back appropriate service-user information to the Registered Practitioner.
