In addition to the main duties of the post above, the following details the key responsibilities of the role:
1. Participate in bi-annual Benchmarking audit scheme and effect and monitor any changes required to provide an optimum service for all users.
2. Make appropriate referrals to other healthcare professionals and accept and assess referrals from other disciplines.
3. Provide colleague training, competency assessment, and support as required for the role.
4. Maintain computerized records using DAWN and Cerner software for warfarinized patients.
5. Be a resource within both the Trust and the Community for anticoagulant advice and information on anticoagulant therapy issues.
6. Use LEAN Thinking principles in the development of the Service.
7. Report incidents and complaints pertinent to the service provided to ensure that appropriate actions are promptly taken to proactively manage incidents/complaints and give feedback to the team leader.
8. Participate in regular risk assessments of the Anticoagulant Service in line with Trust policy.
9. Participate in projects undertaken by Medical teams, Consultant Haematologists, and the Department of Haematology concerning the Anticoagulant Service.
10. Have a deep understanding of relevant databases and IT systems, and develop knowledge and skills in the use of the anticoagulant therapy dosing system (DAWN).
11. Be aware of Patient Group Directives (PGDs) and Trust Protocols relevant to Anticoagulation; review and update as required.
12. Ensure full competencies for the treatment and monitoring of patients requiring anticoagulant treatment.
13. Maintain confidentiality surrounding patient care and treatment at all times.
14. Ensure documentation is completed accurately and within agreed timescales using Trust approved documentation, in line with NMC, HPC, Royal Pharmaceutical Society, and Trust guidelines.
15. Have knowledge of and adhere to Trust policy and professional standards.
16. Demonstrate a professional approach to work, acting in accordance with the Trust 4 Ps pledge, NMC Code of Professional Conduct, and HPC, Royal Pharmaceutical Society regulations at all times.
17. Undertake further training and academic qualifications as relevant to the role and service requirements.
18. Keep abreast of developments in anticoagulant treatment by attending local, national, and international meetings and actively networking with colleagues locally, regionally, and nationally.
19. Participate in local and national committees and working parties.
