The palliative care service is looking to appoint a part-time (8 sessions) Training Fellow to join our existing team. We are keen to work with friendly, enthusiastic people who would relish the challenge of delivering excellent palliative care services within a strong, cohesive, and supportive team.
The post will be based in University Hospital of North Durham and will have elements of acute, community, and care home work.
The post holder will be joining an expanding dynamic service with a great team of experienced Macmillan Specialist Nurses and a Palliative Care support team. The successful post holder will work alongside our key partners across County Durham and Darlington to deliver high-quality specialist palliative care.
The Specialist Palliative Care team consists of a Macmillan Palliative Care Lead Nurse, Palliative Care Consultants, Macmillan Clinical Nurse Specialists, and Macmillan Discharge Facilitators.
Main Duties of the Job
1. To provide clinical assessment and management in palliative medicine for County Durham and Darlington NHS Foundation Trust (CDDFT).
2. To work collaboratively with palliative care consultants (acute and community-based) across the County to ensure the provision of high-quality palliative care consistent with national and Northern England Clinical Strategic Network standards and guidelines.
3. To work as part of the Specialist Palliative Care Team providing support and advice to hospital palliative care teams and clinical staff in acute hospitals in County Durham.
4. To promote a seamless transition across all care boundaries for patients with palliative care needs through collaborative working with primary, secondary, and tertiary care services.
5. To facilitate the rollout of national end-of-life initiatives and developments as agreed with the palliative care services.
6. The exact nature of duties will depend on the job plan agreed with individual applicants.
Job Responsibilities
1. Function as an essential member of the specialist palliative care teams throughout the county.
2. To provide high-quality, flexible, and responsive medical support, assessment, and care and/or specialist advice for patients in County Durham who have complex pain, symptom, spiritual, and psychosocial needs which require specialist palliative care interventions.
3. Working with the hospital palliative care team to support the care of patients in the acute hospitals.
4. Liaison with other members of the multidisciplinary team, patients' families, primary healthcare teams, hospital services, and other specialist palliative care teams.
5. To participate in teaching and educational activities (students, healthcare professional colleagues including junior medical colleagues, patients, and families).
6. To maintain continuing professional development through personal development goals and regular review to support and encourage development.
Person Specification
Qualifications
* MB ChB or equivalent
* Higher medical qualification
* Diploma in Palliative Medicine
Experience
* Ability to work as part of a multidisciplinary team
* Relevant full-time equivalent postgraduate training
* Experience in treating primary and secondary malignant disease, and symptoms in non-malignant conditions
* Knowledge and experience of symptom control
* Audit skills
