SS9 PCN South have an opportunity for a digital and transformation lead to join their team on a full-time basis. The lead will work with the clinical and support teams with the SS9 South PCN to identify opportunities for and lead service improvements. The post holder will plan, co ordinate and directly support the delivery of projects across the PCN. The vision of the PCN is to continue on improving patient access, patient and staff satisfaction whilst optimising the use of technology. This role is pivotal to improving quality of care and operational efficiencies so will require the successful candidate to be motivated and have a passion to deliver excellent service within the practices and PCN. You will work closely with the practice and PCN staff to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered. The main duties will include: Optimise digital ways of working. Support the effective adoption of national and local initiatives. To use data, and improve data quality to. Understand demand, capacity and activity and drive improvements in patient experience of access and operational efficiency. Support population health management. Ensure that digital and operational PCN transformation is embedded in, and aligned with ICS and national strategy. Identify opportunities for digital optimisation and understand the impact for the PCN. Be aware of the national digital agenda and how this impacts the PCN in terms of prioritisation, meeting national and contractual targets. About the Candidate If you are an experienced project manager or transformation lead, looking for an exciting new challenge, then we would love to hear from you. The ideal candidate will have experience of working in an NHS environment. Whilst the following skills and experience are important, we would welcome applications from people who can demonstrate strong transferrable skills: Experience in digital/IM&T project management delivery through the entire project lifecycle from initiation to delivery. Good understanding of the digital agenda within the NHS and social care/care sector. Good understanding of current healthcare policy, relating to primary care and integrated care. Ability to work on own initiative and organise workload, allocating work as necessary. Used to working in a busy environment. Adaptability, flexibility and ability to cope with uncertainty and change. Ability to deliver a vision and a willingness and drive to adapt to NHS targets and business planning, whilst keeping abreast of legislative and contractual changes. Self-motivation and forward-thinking approach. Excellent organisational, management and planning skills, IT skills and communication skills. Ability to implement and manage change effectively.
