Our patient cohort is referred from both acute and community setting. Without immediate access to acute investigations, we use multiple 'Point of Care Testing' systems to support the care of our patients. Our diverse range of staff are skilled in appropriate management of patients, supporting them to remain in the community setting, whilst also escalating as appropriate to ensure their safety. Main duties of the job Work closely as part of the Multi-Disciplinary Team for the care of patients requiring medical and rehabilitation support, predominantly post an acute hospital stay. Responsible for the assessment of care needs, development and implementation of patient centered care. Promoting independence and safe discharge home. Lead in our Nurse led unit, especially during out of hours shifts. Shift work and shift management, working with junior members of staff in all aspects of patient care, participating in training and support.
