Job summary
The purpose of the role of Advanced Nurse Practitioner (ANP) will be to be an integral part of our clinical team, caring for patients with both acute care needs, and where appropriate, where patients require some continuity of care. Interested ANPs and NPs with special interests such as in acute care, frailty and mental health are welcome to apply, however those who feel they are more generalist ANPs and NPs should not be deterred.
As an experienced nurse, you will act within your professional boundaries. You will have skills to be able to provide a first line clinical assessment of care via telephone or online triage and where needed and is best practice, you will review patients for face to face assessment. You will take an initial history and clinical assessment to provide a diagnosis, treatment and evaluation of the patients care. You will demonstrate safe, clinical decision-making and expert care for patients. You will be part of a wider clinical team and will be welcome to ask for support or to escalate situations or health needs which feel out of your scope of practice and learn from jointly caring for patients with the GPs.
Main duties of the job
The aim of the role will be to help the wider Great Park Medical Group provide high quality care to our patient population and enhance patient experience. As part of this role you will support service improvement and feed in where you feel we can provide a better service to our patients or better workforce satisfaction. The role will involve training and development of our wider clinical and non-clinical team.
You will work collaboratively within the multidisciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as and when required. We are looking for highly motivated ANPs and NPs with independent prescribing skills to join our team. The role will require you to provide timely, high quality, person centred clinical care to patients addressing their needs to the highest standard of evidence based clinical care.
The role will involve following up patients who need further assessment, making and receiving referrals and actioning results of investigations and medication requests

Job responsibilities
Competently triage patients via online forms or telephone conversations
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with a minor illness and/or an undifferentiated diagnosis
Assess patients via telephone or face to face, recognising the need to assess patients at the most appropriate place to take into account the healthcare need
Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
Assess, treat and educate patients as required, making use of prescribing experience to prescribe safe, effective and appropriate medication as defined by current legislative framework.
To follow up patients as appropriate dependent on healthcare need and the most appropriate use of resources for our population
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
Make professionally autonomous decisions for which you are accountable
Book diagnostic investigations as indicated.
Refer patients directly to other services/agencies as appropriate
Follow up patients as required by clinical need and guidelines within scope of practice
Undertake the assessment of pathology reports, interpret results and direct for further action as warranted
Skills and Knowledge Essential
1. Experience of using prescribing qualification and carrying out medication reviews within skill set and competencies
2. Clinical examination skills
3. Able to triage patients
4. Communicates effectively with patients and carers, recognising the need for alternative methods of communication to overcome difficult levels of understanding, cultural background and preferred ways of communicating, whilst ensuring the patient is fully informed
5. Able to work as part of a wider clinical and non-clinical team for the benefit of the patient and organisation
6. Management of patients with acute care needs
7. Experience of managing patients with long term conditions and/or frailty or within care homes
8. Knowledge of health promotion strategies and comfortable giving lifestyle advice and signposting to agencies to support patients
9. Knowledge of clinical governance issues in primary care
10. Effective decision making and negotiation skills
11. Ability to work within own scope of practice and understand when to refer to GPs
12. Ability to problem solve whilst working under pressure
13. Excellent organisational skills
14. Accountability of own role and other roles in a nurse-led service
15. IT skills (MS Office and EMISweb)
16. Flexibility to meet the needs of the business
17. DBS required
18. Meet the requirements and produce evidence for nurse revalidation
