City Health Centre, part of gtd healthcare, is a practice located in Manchester City Centre. An excellent and rewarding opportunity has arisen to join the team as a Senior Advanced Clinical Practitioner to work across both primary and urgent care.
The successful Senior Advanced Clinical Practitioner will join a team of highly skilled and compassionate staff, committed to providing excellent and safe patient-centred care for all who access our services.
We are situated above Boots on Market Street, which offers a great selection of cafes, restaurants and shops with excellent transport links. Our Walk In Centre is open 8.00am - 8.00pm, 7 days per week.
The successful candidate will be qualified to MSc level in Advanced Clinical Practice and hold a prescribing qualification. The candidate can be from a Nursing, Pharmacist or AHP background, and will be required to be a competent and safe practitioner with primary or urgent care experience.
This is a full-time position, 37.5 hours per week, shifts are 11.5 hours, 08:00 to 20:00, Monday to Sunday. You will be allocated leadership time.
Main duties of the job
As a Senior Advanced Clinical Practitioner, you will assess, diagnose and prescribe where appropriate. In addition, you will provide advice, care and management for urgent and/or primary care patients either via telephone/video or face-to-face. This may involve the management of long-term conditions and diagnosed or undiagnosed undifferentiated illness. Working flexibly between urgent care and primary care, where appropriate and within a job plan, to provide safe, clinical and expert care.
The clinician will work independently within the organisation's clinical strategy acting as a role model and peer support for junior staff. The clinician working in this role will demonstrate advanced level clinical reasoning, critical thinking and will be key to supporting clinical work across the wider healthcare team to meet the needs of patients.
As a Senior Advanced Clinical Practitioner, there will be an expectation to engage in organisational audit and research to maintain professional practice with feedback to local committees. You will also contribute to the educational forums linking with our learning academy to facilitate, deliver, and receive training. There will be opportunities to attend accredited learning opportunities either in house or via our close links with local universities.

Clinical Practice
* Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
* Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate healthcare service.
* Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Refer and discharge safely ensuring that safety-netting advice is offered.
* Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

Non-Medical Prescribing
* If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes.
* You must maintain your knowledge and skills and prescribe within your clinical competencies.
* The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.
* Attend NMP forum and contribute to the delivery of these.
* Ensure annual appraisal and declarations are up to date.
