This is a full-time position, with working hours Monday to Thursday from 08:45 to 17:15 and on Friday from 08:45 to 17:00.

What you will do:
1. Complete Care Act assessments and devise care plans for individuals aged 67 and over.
2. Conduct Mental Capacity and Best Interest assessments, ensuring the highest standard of care is delivered.
3. Coordinate hospital discharges for out-of-borough hospitals and manage a busy caseload.
4. Attend case conferences and liaise with other healthcare providers as necessary.
5. Stay abreast of current legislation and government policies related to elderly care, including the Mental Capacity Act and DoLS.
The Later Life service in St Helens works with individuals aged 67 and over experiencing age-related frailty and mental health conditions such as dementia. The team is robust, comprising 15 Social Workers and 15 Assessment and Review Workers. As a Social Worker, you will play a vital role in supporting elderly individuals and their families through critical health transitions. Good support structures are in place with regular supervision by team managers or advanced practitioners. Please note that while we encourage flexible working, we do not offer condensed hours. Additionally, agility is key to this role, so a care driver is essential.
