A specialist clinical pharmacist role which will help deliver great patient experience and staff support. This dynamic pharmacy team is a core element, embedded in the following clinical areas: cardiology and cardiac surgery, neurosciences comprising neurology and neurosurgery, head and neck services, trauma and related surgery, orthogeriatrics, vascular surgery and critical care. The post comprises a continuous, one-year clinical rotational programme, designed to steer the individual through the Advanced Level Framework.
The post holder will provide medicines optimisation services to medical and surgical patients whilst integrating into the multidisciplinary team day-to-day.
Main Duties of the Job
You will have clinical pharmacy experience and a diploma; if you are not already a prescriber, we would support you to gain this qualification. You will receive support to develop from our advanced clinical pharmacists, and appropriate training. You will offer ideas and collaborate to improve patient care and experience, and will be a key part of the implementation of service developments within the aforementioned clinical areas.
Your role will include ward work, facilitating processing of homecare prescriptions, participation in core professional commitments (comprising LTHT comprehensive aseptics services, Leeds Medicines Advisory Service, anticoagulation services), governance tasks, support to clinical trials where applicable, promoting patient safety and flow, and supervising rotational junior pharmacists and supporting pharmacy technicians in their emerging clinical roles. You may have opportunities to teach and train other clinical staff groups.

Job Responsibilities
Job Purpose: To provide pharmaceutical care to a group of patients within a clinical area and rotate to a different specialty every 6 to 12 months. To provide cover for other pharmacists within the relevant Clinical Pharmacy Team as appropriate. To support the work of the Lead Pharmacist for the area by assisting in the production of drug-related policies, procedures and financial reports, and by performing horizon scanning activities.
Job Dimensions:
1. Provide specialist clinical pharmacy service to patients and the multidisciplinary team within each rotation, as part of a team of pharmacists working in that specialty being professionally accountable for their actions.
2. Provide drug-related advice and information to patients, consultants and other health-care professionals within a Clinical Management Team.
3. Monitor the clinical use of drugs in patients, advising medical and nursing staff how to optimize drug therapy, reporting prescribing and administration near misses, and risk management issues.
4. Provide education to healthcare professionals within the specialty.
5. Participate in the training of junior pharmacy staff within the specialty.
6. Support the Lead Pharmacist for the area with the production and implementation of drug-related policies and procedures, with the production of financial reports and with horizon scanning activities.
7. Participate in operational service commitments to dispensaries, Medicines Information and aseptic services.
The post holder will be required to work evenings, weekends and Bank Holidays on a rotational basis and participate in the on-call from home pharmacists rota.
Knowledge, Skills and Experience Required
1. Masters qualification in Pharmacy, or equivalent qualification, plus completion of pre-registration training.
2. Pharmacist Registrant of the General Pharmaceutical Council (GPhC).
3. Postgraduate Diploma in Clinical Pharmacy or equivalent experience or qualification.
4. Moderate or significant post registration experience as a practising pharmacist.
5. The ability to use Medicines Information resources.
6. The ability to interpret information and provide advice.
7. Awareness of own limitations and when to refer to others for advice.
8. Dispensing and checking skills ability to follow procedures and accurately supply medication.
Principal Duties & Areas of Responsibility
1. Provide pharmaceutical care to a group of patients in various specialties on a rotational basis. Be responsible for ensuring that the legal and statutory requirements in relation to medicines are met and ensure that the delivery of pharmaceutical care to patients in the defined clinical area meets both national and local standards.
2. Provide professional pharmacist cover on a sessional basis to inpatient and outpatient dispensaries, Medicines Information, Aseptic services and Mental Health, in order to meet legal requirements.
3. Participate in the dispensing and checking of clinical trial materials.
4. Participate in documenting of prescribing and administration near misses (interventions).
5. Assist clinical colleagues in the development and implementation of policies and procedures for the informed, economic use of medicines.
6. Assist clinical colleagues with horizon scanning activities for new treatments and products in the relevant area and with submissions to the Drugs and Therapeutics committee.
7. Assist clinical colleagues in the provision of accurate financial and risk-management data to the appropriate Clinical Management Team for the specialty.
8. Participate in the provision of Education and Training activities for all pharmacy staff.
9. Participate in research and development programs and audit within pharmacy services.
10. Receive prescription payments from patients where appropriate.
11. Participate in the department's late night and weekend working arrangements.

Essential
* Ability to walk moderate distances to wards. Eg: Able to fulfil Occupational Health requirements for the post (with reasonable adjustments if necessary), including clearance on blood borne viruses.
Qualifications Essential
* Pharmacist Registrant of GPhC.
* MPharm or equivalent.
* Postgraduate Diploma in Clinical Pharmacy (or equivalent).
