Service Care Solutions are looking for an Outreach Family Support Worker to join Liverpool City Council's Children & Young People's Services. The role involves delivering strength-based interventions to support children and families facing emerging challenges. You will play a vital role in coordinating multi-agency support and ensuring a whole-family approach to addressing identified needs.
Liverpool City Council are currently looking for someone who is happy to work 35 hours per week.
Responsibilities
* Undertake Early Help Assessments with families to identify strengths and areas for support.
* Develop and implement family support plans tailored to individual needs.
* Chair Team Around the Family (TAF) meetings to review progress and coordinate multi-agency interventions.
* Work directly with children and young people, helping them explore their views and worries.
* Ensure that support plans and interventions are effectively improving outcomes for children, young people, and families.
* Collaborate with partner agencies to provide a holistic, family-focused approach.
Requirements
* Experience working with children, young people, and families in a support role.
* Knowledge of Early Help Assessments and multi-agency working.
* Ability to engage and build relationships with families facing challenges.
* Strong communication, organisational, and problem-solving skills.
* Understanding of safeguarding procedures and family support interventions.
