We are currently seeking a Community Sister to join our Bishop Auckland Community Nursing Team. The successful applicant will work across Bishop Auckland and the surrounding areas from an office base in Bishop Auckland. The team cover 8am until 9pm 7 days a week on a duty roster. The post holder will be a caseload holder to ensure the provision of clinical management to District Nursing and Intermediate Care Services. The role involves providing holistic nursing care to adult patients in their own home or residential care setting. Successful applicants will be required to hold a first level nursing registration with the NMC; additional training specific to the role can be provided.
Main Duties of the Job
To support coordination and delivery of high quality nursing care that, where possible, meets identified patient needs, supports the aims and objectives of the integrated health and social care team, and remains within the boundaries of available resources. Predominantly complex clinical care and caseload management with some coordination and supervisory responsibilities for healthcare staff between Bands 2 to Band 5. The Community Nursing Team provides both planned and reactive care including Urgent Community Response.
Job Responsibilities
The successful candidate will provide a variety of nursing care to patients; including wound care, administration of medication, palliative care, and management of bowel and catheter care. The role involves assessment of patient care needs, care planning, delivery of care, and review. The successful candidate will be required to work independently as part of the wider Nursing team reporting back to the Team Lead. To support patient safety and clinical effectiveness providing expert evidence-based clinical advice and support staff within the team.

* Evidence of recent ongoing nursing development

* 1st Level Registration with the NMC (Adult)

Qualifications
* 1st Level Registration with the NMC (Adult)
* Professional knowledge of community nursing acquired through degree or equivalent level of demonstrable professional knowledge/experience
* Supplemented by diploma level specialist training, experience, short courses which indicate a clear passion for nursing in the community
* Physical skills course or equivalent
* Non-Medical Prescribing
* Management and leadership course
Special Requirements
* Ability to work hours flexibly when service needs require
* Ability to travel independently
