Job summary We have an exciting and rewarding opportunity for a Triage Mental Health Support Worker to join our Health based Place of Safety (HBPOS), 136 Suite based at St George's Hospital, Stafford. You will work with our frontline Community Triage Team and work closely with the local police to offer the least restrictive options/alternatives to detention under the Mental Health Act where possible to patients in need of input from mental services. The 136 Suite is a 2 bedded facility covering the catchment area of Staffordshire, to enable Police to bring people of all ages in who have been detained in the community under Section 135/136 of the Mental Health Act (MHA). They can stay in the suite for up to 24 hours to allow time for a MHA assessment to be arranged, this can be extended for a further 12 hours if necessary. You will greet the patients upon arrival, take handover from Police & paramedics, they will then arrange the MHA assessment with Trust & Social care agencies. During admission staff will assess the patients, arrange physical health assessments, and liaise with other agencies involved with their care. You may also support the Crisis Resolution and Home Treatment Teams (CRHT) with additional assistance and support as required. You will be required to work shift patterns to cover a 24/7 hour/365 day service. Main duties of the job As a Triage Mental Health Support Worker, you will be expected to: Support the place of safety and coordinate assessments with the support from the site manager. Respond in time of crisis by taking direct action through observation & engagement or informing the CRHT worker/site manager of a need for intervention. Liaise with other professionals including police, CRHT, site manager and ward staff. Support service users to develop skills to enable them to integrate into community. Support and signpost to service users in arranging their own finances and other related matters, e.g. housing benefit, rent payments. Work with service users as agreed through care co-ordination or the agreed care plan and to liaise with other professionals, e.g. other members of the mental health team to maintain a consistency of care.
