Role Summary The post-holder will hold an IAPT Post graduate PWP Certificate in Evidence Based Psychological Interventions Be enrolled in, willing to undertake, undertaking or qualified from health coaching training, with the training delivered by a training organisation listed by the Personalised Care Institute. The post holder may also have Psychological Wellbeing Practitioner qualification or a relevant degree (e.g., Psychology with a Post Graduate Cert/Dip in Mental Health or equivalent) or be Experienced and Qualified Graduate Mental Health Worker with relevant experiences and competencies They will manage a caseload and provide Mental Health Care services within St Helens Central PCN, coaching patients both face-to-face and virtually. The role is predominantly hub-based, working across multiple locations, with some remote work and outreach clinics. A driving licence and access to a car on a daily basis is essential. Key Responsibilities Conduct holistic mental health assessments, including rapid screening, clustering, and risk evaluations. Deliver evidence-based step 2 interventions, including psycho-education, guided self-help, cCBT, and workshops across multiple formats (face-to-face, online, and telephone). Provide personalised support and coaching to empower patients in managing their health and wellbeing. Assist patients in setting and achieving health goals while promoting self-management and positive behaviour change. Facilitate referrals to appropriate services, including social prescribing, primary care, and community mental health resources. Support primary care professionals in mental health referrals and interventions. Collaborate with social prescribing teams and external agencies to ensure holistic patient care. Maintain patient records, adhere to clinical protocols, and contribute to data collection. Participate in ongoing professional development, clinical supervision, and training to maintain best practices. Duties and Responsibilities Conduct patient-centred interviews to identify needs, develop action plans, and provide tailored interventions. Make informed decisions on referrals, stepping care levels up or down as necessary. Provide flexible one-to-one and group-based support through various channels. Attend multidisciplinary meetings and liaise with external agencies to enhance patient care. Deliver health education and psychological interventions to individuals and groups. Maintain accurate and up-to-date client records and databases to track progress and monitor outcomes. Perform risk assessments, implementing risk management plans where needed. Contribute to service development by identifying specialist areas of interest and sharing best practices. Ensure adherence to St Helens Central PCN policies, safeguarding protocols, and professional standards. Take responsibility for achieving clinical targets and objectives. Engage in Continuing Professional Development (CPD), attending relevant training and maintaining professional competency. Report workload, progress, and results to Line Manager. Manage time and resources efficiently. Prioritise workload with guidance from Line Manager. Perform additional duties as agreed with Line Manager.
