Job summary We seek an enthusiastic and dynamic pharmacist to work 30-37.5 hours a week in our practice. The hours may be flexible and can include some home working if safe and appropriate to do so. Completion of the CPPE pathway and independent prescribing are desirable but not essential. Two years' experience in pharmacy within the healthcare setting or equivalent is also desirable but not essential. Main duties of the job The post holder will work as part of a multi-disciplinary team with the PCN pharmacists in a mainly patient-facing role. The post holder will take responsibility for areas of medicine management, clinical medication reviews, supporting the general practice staff with regards to prescription and medication queries. They will provide support for the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice, while providing clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
