We offer assessment and nursing care to vulnerable adults. We are known to manage complex challenging behaviours leading to a safe discharge. Elmwood Ward's MDT model includes; Nurses, Health Care Support Workers, Occupational Therapists, Physiotherapists, Activity Co-ordinators, Carer's Liaison, Social Worker, Admission & Discharge Co-Ordinator, Medics and Pharmacy Technicians in order to give our patients the best possible care that we can. Main duties of the job The post holder will be required to contribute where required to the provision of a 24 hour, seven day acute inpatient service. You will work under the supervision of the Ward Manager and Charge Nurses, adhering to the Nursing and Midwifery Council (NMC) Code of Professional Conduct and relevant professional guidelines. You will be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care. You will develop, implement and evaluate care plans in conjunction with patients, carers and other professionals ensuring individual needs are met. You will carry out all relevant forms of care, taking charge of the ward from time to time as allocated. As part of your professional development programme, you will be expected to attend additional training and actively participate in monthly supervision. You will work as an active member of the wider unit team, taking lead responsibility for a key project/ subject matter. You will be required to prioritise attendance at all training, attend supervision and apply reflective practice continuously to enable learning needs to be identified and met.
