Job summary
Arden PCN are looking to recruit a Clinical Pharmacist who is passionate for delivering a quality service to their patients. The ClinicalPharmacist role supports to improve patients health outcomes and of the efficiencyof the primary care team by providing direct, accessible and timely medicinesexpertise, including face to face. Clinical Pharmacist proactively transferworkload relating to medicines optimisation issues to improve patients caresafety and free up clinical staff to spend more time on clinical care.
Location: The successful candidate will be based at Alcester Health Centre, but must be willing to travel across all practices if required to support business needs. Due to the geography and nature of the role a clean UK driving license and full use of car is essential.
Main duties of the job
The post holder will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.
1. To work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using extensive experience and in-depth knowledge to be directly responsible for care and prescribing of defined cohort(s) of patients
2. To deliver and develop pharmacy led medicines optimisation working with general practice and the wider multidisciplinary team, ensuring practice remains up to date
3. To lead and promote, support and facilitate medicines optimisation for specialist area in the wider medicines agenda ensuring best value, reduced harm and improved outcomes.
4. To lead and deliver evaluation and audit of service delivery for specialist area in order to ensure a high-quality service and to further develop provision.
5. To work to the principles of Medicines Optimisation and relevant up to date national guidelines and evidence base.

To improve patients health outcomes and the efficiency of thePrimary Care team by providing direct, accessible and timely medicinesexpertise including face to face. Provide proactive leadership onmedicines and prescribing systems to the PCN multidisciplinary team, patientsand their carers.
Develop and deliver patientfacing services and clinics which improve patients outcomes, reflect the needsof the PCN population and shift workload from other disciplines.
Deliver direct patientconsultations and prescribing as part of the practice and clustermultidisciplinary team approaches acute illness.
Improve patient and carerunderstanding of, confidence in and compliance with their medicines.
Provide pharmaceutical consultationsand home visits to patients with long term conditions as an integral part ofthe multidisciplinary team.
Field medicines and prescribingqueries from patients and staff.
Organize and oversee the PCNSmedicines optimisation system including the repeat prescribing and medicationreview systems.
Deliver high quality medicationreview and develop bespoke medicines treatment plans for patients (particularlythe elderly, complex patients, and those at high risk of admission)
Improve the safety and qualityof prescribing following hospital admissions and attendance
Improve the quality andeffectiveness of prescribing through clinical audit and education to improveperformance against NICE standards, prescribing guidance, and other clinicalstandards.
Develop yourself and the rolethrough participation in clinical supervision, training, and service redesign activities.
Provide a clear and active linkwith community pharmacists and other stakeholders in the medicines supply chainto improve patient experience and manage issue.
Deliver training, monitoringand guidance to other clinicians and staff on medicines issues.
Contribute to the achievementand maintenance of good to outstanding CQC registration status.

Qualifications
Essential
6. GPHC Independent Prescribing Qualification.
7. The successful applicant will have completed the CPPE pathway, or be at least half-way through.
8. Evidence of post qualifying and continuing professional
9. development.
10. Extensive knowledge of medicines optimisation through post graduate diploma or equivalent experience or training.
11. Member of GPHC.

12. Must understand the background to and aims of general practice.
13. Has appreciation of new NHS architecture including the relationship between individual provider, PCN and commissioners.
14. Understanding of the current national policies those impacting on medicines optimisation services.
15. Ability to constructively challenge the views and practices of clinicians and managers.

17. Good interpersonal skills.
