The Hospital Discharge Team are based at the Diana Princess of Wales Hospital. The Hospital Discharge Team is responsible for the safe and effective discharge of patients from hospital into the community and also the prevention of admission to hospital. The Hospital Discharge Team works in partnership with the Acute Trust ensuring effective prevention and discharge pathways are followed. The postholder will provide professional support to those staff working in the Hospital Discharge Team. The team operates 7 days per week working between the hours of 8.00am and 8.00pm., You will be expected to work as part of the HDT team under the direction of the Clinical Lead, to ensure the smooth flow of patients through the team. It is expected that by coordinating administrative tasks, and liaising with other departments, you will contribute to the efficient and effective running of the Discharge Team resulting in a significant reduction in patient journey times and an effective discharge plan being in place.
