North Devon Coastal Primary Care Network
The Clinical Pharmacist will work as part of the integrated practice team and liaise on a daily basis with the clinical and administration team to manage clinical issues as per the duties and responsibilities of the post holder.
The Clinical Pharmacist will have a key role in ensuring the safety and effectiveness of the prescribing at the practice, by working with the repeat prescription team, care homes and other healthcare professionals in a number of roles, including education and prescribing in their own right.
This will include structured medication reviews for patients with long-term conditions or on high-risk medication, active participation in weekly care home board rounds and responding to medication queries from patients and healthcare professionals. Reviewing and updating current protocols and procedures will also be an important part of the role.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will provide primary support to general practice staff with regards to prescription and medication queries, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).

Main duties of the job
Risk stratification, service development, information management, medicines quality improvement programmes, medicines safety, Care Quality Commission, implementation of local and national guidelines, education and training, public health, vaccinations, PCN requirements, differential/undifferential diagnosis, patient-facing medicines support, telephone medicines support, extended hours, medicines information to practice staff and patients, frailty reviews, management of medicines including reconciliation for discharges, new patients and recent specialist appointments, repeat prescribing.

The North Devon Coastal Primary Care Network was formed in June 2019; the partnership comprises Combe Coastal Practice, Caen Medical Centre and South Molton Medical Centre all within North Devon. Between the network, we look after approx. 45,000 patients across these 3 Practices.
Whilst retaining our independent practices, we are working together on collaborative projects to improve and develop the healthcare services we offer to patients.
The post holder will be part of an expanding pharmacy team which currently includes 4 pharmacists and 2 pharmacy technicians.

Qualifications
* Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
* Has an independent prescribing qualification
* Member of the Royal Pharmaceutical Society (RPS)
* Hold a post-graduate clinical diploma
Experience
* Experience of working with patients in a busy environment
* Experience of working in Primary Care
* Experience in the use of Primary Care/NHS computer systems
