To work within both the Continuing Care and Reactive Care team to;
Deliver evidence based nursing care to children and young people (CYP) in a variety of community settings by providing clinical assessment, planning, delivery, evaluation of care and clinical advice in liaison with the team leader, peers and or multi-disciplinary professionals.
Recognise and monitor acute exacerbations of illness and initiate appropriate nursing support and action any urgent medical interventions.
Work collaboratively with the hospital inpatient areas, accident and emergency, A&E, Observation and Assessment unit and primary care to facilitate early and prompt discharge and deflect unnecessary admissions to secondary care.
Collaborate with the multidisciplinary team and other service providers to ensure care is patient centered, efficient and effective and to develop guidelines and policies for children with acute and continuing health care needs.
To work preventatively utilising health promotion skills and implement self-management skills initiatives / plans for CYP and families to ensure optimum health outcomes.

To act as patient’s advocate in promoting, and delivering the highest standard of nursing care, ensuring all fundamentals of care are addressed and current, relevant policies and procedures are adhered too.
The post holder will work as anautonomous nursing practitioner to support named nurses for designated caseload for CYP and families. The Senior nurse will work collaboratively with other professionals and agencies in the assessment, implementation, evaluating specialist care to children with(long term, life limiting or palliative care conditions).
