Community Heart Failure Clinical Nurse Specialist
The role of the Heart Failure Nurse within thecommunity has been established in order to meet the needs of people with heartfailure in the Surrey locality is to optimise the management of people withheart failure, reducing unnecessary hospital admission and providing a linkbetween the GPs and specialist teams. The post holder will offer education andsupport to health care professionals, people with heart failure and those whocare for them.
Main duties of the job
Responsiblefor communicating highly sensitive information and bad news to patients andcarers in an empathetic and supportive manner empowering the patient to managetheir condition e.g., development and impact of complications, effect onactivities of daily living etc.
Responsible forteaching patients and their carers to manage their heart failure to the best oftheir abilities. The aim being to prevent short term and long termcomplications, this may be one to one or in groups.
Communicateseffectively with patients and carers to ensure a clear understanding of heartfailure and its management. This will involve providing and receiving complex,sensitive information and will require developed motivational, empathetic andreassurance skills. There will be barriers to understanding.
Acts as an advocatefor patients particularly in residential homes, ensuring that they receive theunderstanding and facilities they need to manage their heart failure.
Responsible formaintaining and developing effective joint working relationships with nursingand medical colleagues throughout partnership working
Responsible forproviding advice and information on heart failure, that may be complex, tomedical and nursing colleagues and the general public.

Experience
* Up to date knowledge of current clinical and professional issues
* BLS
Knowledge and Skills
* Good organisational skills
* Good time management
* Ability to work independently unsupervised
* Good computer and keyboard skills to use excel
* Good communication skills both written and verbal
* Counselling skills
Qualifications
* RGN or equivalent HCPC qualification
* BSc or equivalent qualification in profession
* Advanced Clinical Assessment skills
* ENB 998 Teaching and assessing or equivalent
* Independent non medical prescriber V300
* Qualified for 5 years with 2 years cardiac nursing experience
* Evidence of CPD at Level 7
