Introduction
BCVS is a highly respected Local Infrastructure Organisation (LIO), working in a large geographical district covering the northern third of Nottinghamshire and Bolsover. The breadth of our strategic work spans the NHS ICS areas of South Yorkshire, Nottingham and Nottinghamshire and Derby and Derbyshire, plus the Local Authority areas of Nottinghamshire and Derbyshire. BCVS actively represents the VCSE sector at over 70 meetings and forums, and has been a key player in the Bassetlaw and Bolsover response to the impact of COVID-19.
BCVS is a forward-thinking and proactive LIO that delivers a range of projects and core infrastructure support for the VCSE sector. A key strength is its collaborative and partnership-based approach that has brought clear outcomes for the VCSE sector and the communities it serves.

Purpose of the Job
Social prescribing link workers connect people to community-based support, including activities and services that meet practical, social, and emotional needs that affect their health and wellbeing. This includes connecting people to statutory services for example housing, financial and welfare advice. They work collaboratively across the health and care system, targeting populations with the greatest need and risk of health inequalities.
The Social Prescribing Link Worker is a key part of the primary care network (PCN) multi-disciplinary team in a complex, ever-changing environment. Voluntary and community involvement is becoming more critical in demand management, self-care and the wider health and wellbeing of the community of Bassetlaw.
As such, the role requires someone with strong inter-personal skills, sound knowledge of the voluntary and community sector and social care environment, and someone who is both well organized and capable of operating at strategic and operational levels.
Key Responsibilities and Deliverables
1. Respond to incoming referrals from a wide range of agencies, offering a bespoke service of support to navigate systems to meet individuals’ health and wellbeing needs.
2. Support and, where appropriate, lead project work within BCVS and local partnerships.
3. Effective regular liaison/communication with Newgate Practices and statutory partners, providing a two-way link with the voluntary and community sector.
4. Educate non-clinical and clinical staff within their PCN multi-disciplinary teams on available community services.
5. Provide personalized support to service users, their families and carers to take control of their health and wellbeing.
6. Manage and prioritize your own caseload, ensuring appropriate referrals to other health professionals/agencies when necessary.
7. Draw on and increase the strengths and capacities of local communities, supporting development of local voluntary organisations.
8. Work collaboratively with all local partners to contribute towards supporting local CVS organisations and community groups.
9. Raise awareness of social prescribing and its benefits.
Key Tasks
Referrals
* Build relationships with staff in GP practices within the PCN and attend relevant MDT meetings.
* Promote social prescribing and encourage appropriate referrals.
* Connect with local communities, particularly those that statutory agencies may struggle to reach.
Provide Personalised Support
* Meet patients on a one-to-one basis, including home visits.
* Help patients identify wider issues impacting their health and wellbeing.
* Work with individuals to co-produce a simple personalised support plan.
* Consider the support needs of patients’ families and carers.
* Offer a “hand holding” service, introducing patients to community groups.
* Explore the option of Personal Health Budgets for eligible patients.
* Seek advice and support from the GP supervisor for patient-related concerns.
Support Community Groups and CVS Organisations
* Maintain strong links with relevant voluntary and community sector organisations.
* Develop supportive relationships with local CVS organisations and statutory services.
* Support the maintenance of Bassetlaw CVS information website and internal CRM.
Work Collectively with Local Partners
* Identify unmet needs within the community and gaps in provision.
* Encourage patients connected to community support to volunteer and provide peer support.
* Attend relevant BCVS internal and external events/meetings.
* Provide regular feedback to community groups receiving referrals.
General Tasks
* Work sensitively with patients to capture key information.
* Encourage feedback and document case studies.
* Work closely with GP practices to ensure proper data entry and reporting.
* Engage in continual personal and professional development.
* Adhere to organisational policies and maintain confidentiality.
* Work as part of the healthcare team for continuous service improvement.
* Flexibility is required within the remit of this post.
Current Meetings / Committee Responsibilities
* Involvement in practice MDTs as required.
* Involvement in PCN meetings as required.
* Involvement in the ICB engagement forums as appropriate to the role.
