Join to apply for the Cross Gates PCN Advance Nurse Practitioner role at South & East Leeds GP Group Limited.
The position is for 19 hours per week.
You will work within the PCN multidisciplinary team and be expected to undertake acute and planned visits to patients in their own homes and those living in residential and nursing homes. You will be able to appropriately assess, examine, investigate, diagnose and treat a wide range of complex or urgent conditions, resulting in the safe management and appropriate referral of patients with undifferentiated, undiagnosed and complex presentations. You will also support practices with on the day demand by delivering acute clinics for patients across all four practices.
The Post Has Two Main Functions:
1. To carry out the duties as outlined.
2. To be proactive in continuously improving service delivery.
Main Duties of the Job:
Clinical Responsibilities:
1. To assess and triage patients, including same day triage, and as appropriate provide definitive treatment including prescribing medications following policy, patient group directives, NICE national and local clinical guidelines and local care pathways or make necessary referrals to other members of the primary care team for patients unable to attend the surgery.
2. To work at an advanced level of practice within PCN policies and procedures.
3. Manage a range of patient long-term conditions including but not limited to Asthma, COPD, Diabetes and CHD following practice guidelines.

Cross Gates Primary Care Network includes four practices working across seven sites. The PCN has a patient population of 30,000 including patients living across 5 care homes.

Salary:
£28,407 to £34,581 a year Depending on experience.
Contract:
Permanent, Part-time, Flexible working.
Person Specification:
Experience Essential:
* Significant experience of working within a social care and/or healthcare/ virtual setting.
* Competent in the assessment, planning and management of complete clinical episodes of care for patients, working autonomously.
* Experience in leading a team and managing staff within the team.
Qualifications Essential:
* Educated to masters degree Level in relevant area of expertise.
* Advanced Clinical Practice qualification at either degree postgraduate level or masters level.
