Job summary This is quite a new role in a relatively new PCN organisation, so were looking for someone with the flexibility to shape with us the support we need hence the wide scope and flexible nature of this role. This is so much more than a traditional Pharmacist role as it covers critical areas of Pharmacy work in our GP Practices. The Pharmacy First support role is to be carried out over a minimum of 3 clinical sessions per week. This document outlines the overall objectives for the Pharmacy Lead role that has been introduced for 2024/25 to form part of the overall Change Management Programme supporting General Practice. This document gives an overview of the roles and responsibilities of the pharmacy lead to support the development and uptake of Pharmacy First, in line with the Primary Care Access Recovery Plan (PCARP). Supporting practice staff to engage with change, troubleshooting, identifying support needs, signposting, and escalating as appropriate. Contribute to performance improvement, by supporting any practices experiencing significant issues, taking a lead for identified areas where agreed. Attend and participate in relevant internal and external working groups and provide project advice, expertise, and support, where requested. Attend and update at relevant meetings within their boroughs such as practice manager forums, clinical director forums, and other relevant groups. Build and maintain relationships with Place-based primary care teams to ensure that information is being shared and key stakeholders are informed of the development and progress of the primary care strategy for SWL Act as the liaison with local stakeholders to better understand what their needs are and support the capture and sharing of good practice, developing case studies, and generating ideas and activities that increase impact locally. Support pharmacies and practices to develop and engage across their PCNs by identifying key stakeholders and developing relationships and communication with clinical directors and pharmacy lead ARRS staff. Working with local training hub leads to hosting training sessions for practices and developing a robust network of digital champions to support pharmacy first. Work in an integrated manner with the SWL Primary Care Transformation team, Training hub leads and colleagues from Redmoor Health to deliver effective system-wide transformation projects.
