gtd healthcare is one of the largest NHS commissioned Primary and Urgent care providers in North West England, we have a number of excellent part time and flexible working opportunities for Care Co-ordinator roles to support our patient services in Greater Manchester.
Our services operate 24/7 providing vital patient care from Ashton Primary Care Centre, Manchester Royal Infirmary, North Manchester General Hospital, Wythenshawe Hospital and Denton Clinical Hub.
We are recruiting flexible, part time roles, up to 30 hours a week, across midweek evening and overnight shifts and weekend daytime, evening and overnight shifts.
Care Co-ordinators are the first point of contact for health care professionals, patients and relatives of patients accessing NHS urgent and non-urgent care services. They are pivotal in the smooth running of our services and ensure patients are directed to the most appropriate healthcare service or pathway.
Main duties of the job
The role of the care co-ordinator is fundamental to the smooth running of the out of hours and urgent care service.
Duties include:
* Answering the telephone
* Reception
* Corresponding via emails and text messages
* Signposting to relevant services
* Basic reporting
* Driving (if over the age of 25 & less than 3 driving points)

Job responsibilities
As a Care Co-ordinator for gtd healthcare, you will be the first point of contact for health care professionals, patients and patients' relatives accessing urgent and non-urgent care services across a number of integrated care board areas.
Under the direction of the senior care co-ordinator and clinical shift lead, you will register patients onto the relevant clinical system and direct them to the most appropriate service or pathway contributing to a 24-hour, seven-day-a-week Care Coordination Service. This is a multi-skilled role and will include call handling, treatment centre and home visiting (where able) elements.

Key duties and responsibilities include:
* To deliver a high quality care co-ordination service to patients and health care professionals
* Accurately record on a range of clinical systems all relevant data for each contact and direct appropriately
* Where demographics are incomplete, complete all necessary checks e.g. NHS Spine to ensure most up-to-date records are available
* Undertake the booking of appointments across a number of services in line with agreed timescales
* Provide a first class reception service, ensuring a professional meet and greet, efficient registration and onward referral to the relevant area/service
* Liaise with the senior care co-ordinator and clinical shift lead regarding any operational issues affecting the flow of patients and ensure excessive waiting times do not occur
* Follow the failed encounter procedure for all patients that did not attend (DNA) for a face-to-face assessment
* Coordinate the arrangement of any follow-up care as directed by the senior care co-ordinator or clinical team
* Liaise with the multi-disciplinary team to ensure the smooth running of the service
* Liaise with NHS 111 service regarding patients who are out of area, duplicate calls or incomplete information
* Using one of the company vehicles, ensure the safe transit of medical and non-medical colleagues undertaking home visiting shifts in a timely manner working to set KPIs (this is applicable only to those who are over 25 with a valid, full UK driving license due to insurance policies)
* Complete basic checks of the fleet vehicles to ensure they are roadworthy
* Comply with The Highway Code, road safety and vehicle rules
* Ensure all equipment and medication is checked at the start and end of shift and report any discrepancies to the senior care co-ordinator
* Facilitate the transfer of electronic data both internally and externally ensuring that information governance rules are followed at all times
* Have a full understanding of the clerical backup system to facilitate the smooth transaction from a computer-based system to a paper-based system
* Fully comply with the Complaints Policy ensuring all informal and formal complaints and incidents are documented and the process is explained to patients
* Responsible for undertaking administration duties within the service
* Ensure regular attendance at operational meetings
* All training deemed as mandatory to be completed within the specified time frames
* Perform any tasks as requested that are within the boundaries of the role
* Ensure that all gtd healthcare policies and procedures form the basis of all decision making

* Good standard of education
* NVQ Level 2 in Health Care
Experience
* Excellent telephone manner
* Evidence of working to defined policies and procedures
* Experience of using a computer
* Experience of a busy telephone answering environment
* Experience of working with the public
* Experience of liaising with multi-disciplinary agencies
* Primary care experience
* At least one year's experience of reception or call handling duties
