Job summary The Band 3 Community Coordinator provides support to the wider community team by processing and organising patient discharges on a daily basis, ensuring patients receive seamless care by ensuring appointments are made using the digital system and dealing with patient enquires. The Community Coordinator serves as a point of contact/support for women and their families and they provide appropriate information and, where necessary, signpost to wider support organisations. The post holder will demonstrate effective application of problem solving and planning skills and a broad skill base related to their practice. They will need to have excellent communication, administrative and organisational skills, with the ability to liaise with employees at all levels. Knowledge of Microsoft Office, Word and Excel are essential. The post holder will be based primarily at the Bedford Community Hub, however, may be expected to travel to Bedford Hospital and other Community Hubs as part of the role. Main duties of the job To take responsibility for the day to day organisation of the community discharges. Ensuring data is entered on to the electronic workbook and ensuring all staff are aware of their workload for the day. Having an oversight of the workbook, ensuring this is kept up to date and follow up appointments are in place for all women. Ensuring clinic lists are kept up to date and changes made when requested to do so. To make appointments in line with agreed care pathways and guidelines. To follow up on women who DNA, in line with agreed care pathways and guideline. To receive telephone calls and action these appropriately, escalating to a midwife when required. To access and action messages left by patients on a daily basis. Ensure effective communication with colleagues, women and their families. To support the wider community team with ordering or arranging repairs of equipment. Liaise effectively with the clinical teams within the Maternity services Contact patients to arrange appointment dates in accordance with the Trust's Booking Policy. Management of monthly returns including mileage forms, expenses and ensuring any patient referrals are actioned including teenage pregnancy forms, smoking cessation and referrals into the tongue tie service. To escalate any urgent problems to the Midwifery Leads and Service Managers Undertake necessary training as and when required by the Trust.
