Job Purpose: To provide assessment and management of patients who have osteoporosis and bone health conditions. To work autonomously delivering high quality evidenced based care to patients seen in their own home or in a care home setting. To facilitate and autonomously deliver osteoporosis therapies in the community. To work in partnership with GPs, Community Geriatricians and Specialist Secondary Care Physicians in the management of patients within Nottingham City who have osteoporosis and bone health conditions.
Dimensions: The post holder will be part of the citywide specialist Bone Health team who are based at Bulwell Riverside. The post holder will administer IV and injection therapy in patients' homes and care homes and will be expected to spend 80% of their clinical time completing patient facing activity. The post holder will report directly to the Band 7 Bone Health Nurse Specialists. The post holder will be able to fulfill physical demands of the role, including manual handling and be able to travel city-wide for visits, meetings and further if required in response to the needs of the service.
Key Responsibilities:
1. Clinical: To be professionally and legally responsible and accountable for all aspects of work, including the management of patients and to ensure a high standard of evidence based clinical care for patients. To work autonomously within a patient's home or care home. Effectively prioritise, assess, plan, deliver and evaluate care. To safely deliver the community IV therapy and injection service in patients' own homes and care homes.
2. To accept clinical responsibility for a designated caseload of patients and to organise this effectively regarding clinical priorities and use of time.
3. To act as a link between primary (GPs) and secondary care (Specialist Physicians) to deliver seamless care for patients within the specialist area.
4. To participate in the Bone health Virtual Clinics with the community specialist Consultant.
5. To attend case conferences/CDG meetings within the link neighbourhood areas that have been allocated.
6. To participate in the specialist team's audit and research projects.
7. To always ensure the health & safety of staff and patients in accordance with Health & Safety legislation and Trust policy and procedures.
8. To adhere to all other Trust Policies and Procedures.
9. Contribute to the Clinical Governance agenda and apply risk management systems.
10. Complete Incident Reporting forms when required.
Professional: The post holder would need to demonstrate a commitment to evidence-based practice and to effectively discuss management plans with Bone Health Specialist Nurses. To be responsible for own Continuing Professional Development keeping up to date with new research and developments in the specialist area, incorporating them into their work as appropriate. To attend relevant conferences and internal/external training courses in the specialist area if required.
To provide evidence of ongoing and appropriate CPD and to participate in the organisations PDR process.
Leadership: To participate in the identification of data collection and statistics and to maintain clear, concise, and timely records in line with the service policy. To undertake clinical audit to monitor best practice within the specialist area and to measure these against national and local guidelines in best practice.
