Please refer to the attached job description and person specification for full details of responsibilities. To work as part of a Learning Disability multi-disciplinary, integrated team. To support service users with a range of priority health needs under the supervision of a health practitioner. To participate in providing health duty with the support of health clinicians. Participate in regular planning and review meetings with the health practitioner supervising pieces of work and maintain quality and management of caseload within supervision process Maintain adequate records as required by existing procedures, entering appropriate details on the service users case notes as necessary. Attend and participate in wider team/care plan review meetings as appropriate. Work with service users to facilitate access to primary and secondary healthcare services. Help to identify early signs of mental and physical health decline by monitoring the service users progress, level of functioning and mental state and alert the health practitioner involved in the individuals care. Able to access the local area as this is a community based post which will involve a high amount of travelling.
