The successful candidate will be part of a dynamic and forward-thinking PCN, Clinical leadership will be provided by the PCN Clinical Director and Clinical Leads. We are looking for individuals who will lead clinical care home rounds & coordinate the work of healthcare professionals and non-clinical staff in the care of patients registered at GP practices within the PCN population. The role includes responsibility to work with the practices to support and ensure compliance with the reporting requirements of the PCN Directed Enhanced Services (DES).
