We would like to recruit highly motivated band 6 Midwives to lead the Young Parent Maternity Service within our welcoming, supportive and friendly Enhanced Support Midwifery team. This will be involve working across central Lancashire with young people and their families. The role is a fixed term contract for a period of twelve months.
You will provide antenatal and postnatal continuity of carer to young peopleand act as an advocate for young people and their families tocoordinate care based on their individual needsoptimising mental as well as physical health.
You will work in partnership with a multi-agency and multi-disciplinary team from midwifery, health and social care to provide and co-ordinate the highest standards of care for young people and their families some of whom may have complex social issues.

️ You will perform a key role within the enhanced support midwifery team. Acting as the link between the maternity service and other healthcare, educational and support services involved in the young person’s care planning.
You will work closely with the enhanced support midwifery team to ensure maternity care is responsive to the young person's needs.
You will work autonomously, being responsible for managing a caseload and ensure young people are referred appropriately and properly safeguarded.
You will have a sound based knowledge and act as a resource to colleagues to enable them to provide holistic care that optimises the health and wellbeing of young people and their families with/without complex social needs including mental health issues.

⚕️ Communicate effectively with young people and their families to provide informed choice and personalisation, recording agreed care plans in their electronic patientrecord.
You will work as part of the multi-disciplinary team to provide individualised plans of care for young people and their families
You will develop and deliver training with the support of the specialist midwives, being knowledgeable and supportive of the continuity of carer national agenda.
You will work in partnership with a multi-agency and multi-disciplinary team from health and social work to provide high standards of care for young people with varied complex social needs.
You will act at all times in a manner that upholds the Trust’s values and Nursing & Midwifery Professional Framework goals, working as part of a team to ensure that women and relatives receive excellent care with compassion.
✍️You will participate in data collection to demonstrate the positive impacts of continuity of carer for young people in improving health and pregnancy outcomes.
